Approve the Resolution Transferring a Certificate of Public Convenience and Necessity from Positive Mobility, Inc. D/B/A Elite Medical Response, LLC. to Positive Mobility, LLC D/B/A as Elite Medical Response

 

SUMMARY:

Resolution- COPCN Provider Name Change POSITIVE MOBILITY, LLC D/B/A AS ELITE MEDICAL RESPONSE 

 

BACKGROUND:

Positive Mobility Inc., D/B/A Positive Medical Transport, Inc. was granted the Certificate of Public Convenience and Necessity by the County on December 13, 2022 to provide non-emergency transport of DeSoto County patients to out of county destinations, for an initial term of three (3) years commencing on January 1, 2023 and ending on December 31, 2025, which may be subject to one (1) year renewals at the end of that period with a maximum of three (3) extensions at the discretion of the County.

In September 2023 Positive Medical Transport, Inc. began doing business as Elite Medical Response, LLC.

In April of 2025, Positive Mobility, Inc. D/B/A Elite Medical Response, LLC changed names to Positive Mobility, LLC D/B/A as Elite Medical Response, and has requested that the existing Certificate of Public Convenience and Necessity be transferred to the new entity name.

Recognizing the name change is a requirement by the State of Florida to maintain and operate under the COPCN designation.
